Abstract

In this chapter, the ‘anorexic voice’ (AV) is explored: a pervasive internal dialogue experienced by many with anorexia nervosa (AN). The AV is discussed as both punitive and identity-shaping, mirroring traumatic early relationships and reinforcing anorexic behaviours. Drawing on models such as PAARS-ED, the chapter situates the AV within broader processes of emotional regulation and self-concept. Qualitative accounts highlight its dual function: initially offering comfort or control, but ultimately becoming hostile and entrapping. Clinical implications are considered, including how therapists can engage with the AV without reinforcing its dominance.
