Abstract
A case report describing recurrence of non hodgkins lymphoma presenting as a chronic non healing wound of two years, describing its diagnosis and management. 71 year male presented two years ago with a swelling of the left inguinal region lymph nodes, due to localised diffuse large B cell non hodgkins lymphoma (NHL) associated with night sweats; stage 1B. After diagnosis he underwent surgery, followed by chemotherapy and regional radiotherapy. Despite treatment the surgical site showed no signs of healing. He had a past history of rheumatoid arthritis, systemic lupus erythematosus with vasculitic leg ulcers, non functioning pituitary macroadenoma and Chron’s disease requiring long term treatment with azathioprine and steroids. Examination revealed a large open inguinal wound, with tissue oedema and lymphorrhea associated maceration of the surrounding skin. The wound margin was over hanging and appeared violaceous and thickened. A biopsy of the wound margin confirmed the recurrence of diffuse large B cell lymphoma displaying non germinal centre immunophenotype. Further investigations confirmed advanced disease and patient died within nine months of recurrence. Diffuse large B cell lymphoma (DLBCL) is the most common form of NHL. Typically presents as a rapidly enlarging mass at a nodal or extra nodal site. In this patient immunocytochemistry revealed pleomorphic infiltrate of B cell phenotype. DLBCLs are aggressive tumours that are rapidly fatal without treatment. With intensive combination chemotherapy, 60 to 80% of patients achieve a complete remission and 40 to 50% are cured. This case highlights the importance of tissue biopsy in cases of non-healing wounds.
