Abstract

Background: A key element in the management of spinal pain is the use of reliable, easy to use outcome measures. Postural retraining is one of the elements used in physiotherapeutic management of spinal disorders. Clinicians need outcome measures that will give accurate measurements of posture that can be used to monitor effects of treatment. This study aimed to assess the reliability of digital analysis of thoracic angle, neck angle and head tilt using one measurer within one day. Methods: Twenty healthy control subjects were recruited. L4, C7 and tragus were marked on the skin and identified with reflective markers. The subject sat in a relaxed comfortable position in a chair and video recorded in a frontal plane for one minute. The markers were removed and the subject rested for a few minutes. This was repeated twice more in the same day. Still images were taken at 30seconds of the recording and were analysed using MATLAB software. Results: Digital analysis using MATLAB software has excellent reliability as identified by intraclass correlations of 0.94 (mean difference 0.34° ±4.7°), 0.91 (mean difference 1.1°±3.7°) 0.84 (mean difference 0.9°±4.9°) for thoracic angle, neck angle and head tilt respectively. All points, except one for neck angle and head tilt and two for thoracic angle, were within 95% limits of agreement. Conclusion: Digital analysis using MATLAB is a reliable way to measure thoracic angle, neck angle and head tilt. This is an inexpensive method for measuring posture in the management of people with spinal pain.
