Abstract

In this chapter, the authors describe the recent progress in the research on diffusion-weighted magnetic resonance imaging (MRI) in Alzheimer's disease (AD). We first present an introduction to the clinical and neuropathological aspects of AD, and the current efforts to develop neuroimaging biomarkers, including structural and diffusion-weighted MRI, and positron emission tomography (PET). We then describe the fundamental concepts and the most common quantification approaches for diffusion-weighted MRI imaging (or diffusion-weighted magnetic resonance imaging [DWI]), followed by sections reviewing DWI studies in white and gray matter in AD, respectively. The chapter concludes with a section on key takeaways, limitations, and pathways towards the future, with a special focus on the potential usefulness of DWI for disease-monitoring in clinical trials and its prognostic value in longitudinal studies.
