Abstract
Background: Recent prevalence estimates for hidradenitis suppurativa (HS), compared to previous population estimates in the United States (US), are not well established. Methods: This retrospective population-based study used Optum CDM (through 2024) and Merative MarketScan (through 2023) administrative claims data. Prevalence per 100,000 persons was calculated by dividing number of all prevalent HS cases by number of all eligible persons with continuous coverage throughout a calendar year. Cases were defined as having ≥1 inpatient or ≥2 outpatient diagnosis codes on different days. Sensitivity analysis defined cases as ≥1 inpatient or outpatient diagnosis. Prevalence was standardized to 2024 US Census and pooled across databases. Results: Between 2018-2023, a 107% increase in pooled overall HS prevalence (221 per 100,000 in 2023) was reported, and consistently observed across insurance types, sexes, races, and age groups: 2023 prevalence was 339 (increased 109%) for women and 101 (increased 98%) for men; age group 30-39 years (412, 104% increase) and age group <12 years (3.6, 65% increase) had the highest and lowest prevalence, respectively; age group 40-49 (365, 131% increase) reported the largest increase in prevalence. In Optum, with self-reported race, Black and Asian patients demonstrated the highest and lowest prevalence (446 and 108 in 2024, respectively), while White patients reported the largest increase of 135%. Similar trends were observed in sensitivity analyses. Conclusions: HS prevalence more than doubled between 2018 and 2024 in the US. These data may reflect improved disease awareness and subsequent diagnosis, as well as more accurate coding, reflected in claims data.
