Abstract
Infertility affects one in six couples, many of whom pursue medically assisted reproduction (MAR). Although most patients undergoing MAR have multiple unsuccessful treatment cycles, fertility care has historically focused on reducing distress and minimising dropout, rather than supporting the complex decision making required across protracted treatment trajectories. Recent research highlights a shift towards an integrated care model that emphasises proactive planning and support for an empowered end of treatment, rather than treatment persistence alone. This shift reflects recognition that MAR commonly involves repeated cycles and sustained uncertainty, requiring psychological support tailored to different stages of the treatment journey. Emerging evidence suggests that treatment discontinuation can occur either in the context of emotional, financial, or physical resource depletion, or through a more empowered process characterised by patient agency and intentional decision making. Although several promising interventions have been identified, more work is needed to refine their implementation, address equity-related barriers, and evaluate long-term outcomes such as decision quality, regret, and life satisfaction.
