Abstract

Insulin therapy has been the mainstay of treatment for type 1 diabetes (T1D) for over a century. Despite significant advances in technology leading to increasingly sophisticated methods of delivering insulin therapy and monitoring blood glucose levels, many individuals with type 1 diabetes continue to face significant barriers to optimising glycaemic control. As a result, morbidity and mortality amongst individuals with type 1 diabetes remain unacceptably high. It is clear that alternative approaches to the treatment of type 1 diabetes are required. In recent years, there has been a particular focus on the development of treatments aimed at modulating the autoimmune process that drives the development of the disease, with the aim of preserving beta cell function and enhancing endogenous insulin production. Such treatments may not only be beneficial in individuals with confirmed T1D but are likely to have utility in individuals with beta cell autoimmunity but who have not yet been diagnosed with T1D, as a means of delaying or even preventing the development of clinical T1D and the need for exogenous insulin therapy. This chapter will describe the natural history of T1D, outline strategies for identifying individuals with preclinical stages of T1D and summarise the evidence for the use of a range of interventions at the different stages of disease to delay or prevent disease progression.
