Abstract

Advance care planning (ACP) is a well-established process to support people and care providers to explore their understanding about the individual's illness, values, goals and preferences about places of care and types of treatment. However, this approach has its limitation. Future care planning (FCP) has been evolving both in the UK and internationally. FCP recognises and encompasses the essential elements of ACP, whilst adopting both a values-based and person-centred approach, taking individuals with diminished and/or fluctuating mental capacity into account. Digital solutions have provided an opportunity to support FCP processes, and improve decision-making as well as sharing of views, wishes and preferences regarding care. Here, we outline and discuss current definitions of FCP, describing some of the policy developments where FCP has been formally adopted, and explore an example of how digital solutions have helped improve connectivity, sharing and involvement of important patient updates, including specific care plans.
