Abstract

Background: Advance & Future Care Planning in Wales has been given a more prominent position, and has been supported by Welsh Government and a specifically appointed national strategic group. Method: In this session, some of these national approaches are outlined. Challenges, in particular during the Covid-19 pandemic, are discussed. We outline some of the recent critiques of advance care planning, including its definitions, concerns over blanket approaches and how an impact assessment was conducted regarding protected characteristics. Results: We discuss culture changes in DNACPR decision making and communication, observing that patients who hold such forms can still receive maximum healthcare interventions as part of their day-to-day care. Discussion: Overly outcome-focussed approaches in Advance Care Planning are likely to be misunderstood and misapplied. Rather, we put forward strategies on how a meaningful offer of such a discussion gives patients and their significant others the choice to explore the topic further and ask questions, or indeed state that they do not want this. Data from a national Welsh conference involving patient and carer representatives will be presented. Media reactions to advance care planning will also be discussed as part of this plenary, as well as out-of-hours considerations and novel approaches to teaching.
