Abstract
Objectives To validate the reliability of Malay version of the Mini-Addenbrooke's Cognitive Examination (M-ACE) in Malaysian population to detect the mild cognitive impairment (MCI) and dementia in comparison to M-ACE English version as gold standard. Methods A cross-sectional study involved 132 bilingual consenting patients was assessed using Malay and English versions of M-ACE at Pulau Pinang Hospital, Malaysia. The cut-off score of MCI is ≤ 21 and for dementia is ≤ 16 points (Total score= 30). The English version of the M-ACE was pre-translated and adapted into Malay and then back-translated into English by three blind independent experts. For the five scales of the tool, reliability and test-retest reliability were estimated. Means of the differences between test-retest scores were compared. IBM-SPSS (V22.0) was used for data analysis. Results The age of participants was (60.4 ± 7.7) years, (65.6%) were males. Internal reliability was very good (Cronbach's alpha: 0.803), Cronbach’s alpha for English version was 0.83. Student t-tests showed non-significant differences between the mean scores obtained from the test-retest for each of its five domains (P-value: 0.727). Concurrent validity, estimated by the correlation between total scores of test and retest was significant (P <0.001). The inter-rater reliability was excellent (Cohen's kappa: 0.99). The mean of the differences between test and retest scores was (0.06 ± 0.043). With a cut-off score of 21 points, sensitivity was 96.3%, specificity 95.1%, and accuracy 95.6%. Positive and negative likelihood ratio were 19.65 and 0.04 respectively, for the detection of cognitive decline compared with M-ACE English version as the reference standard. Conclusions The Malay version of the M-ACE is a brief, reliable and very useful screening tool for the assessment of cognitive deficiency and has shown to discriminate between MCI and dementia. The M-ACE showed high sensitivity, specificity, and accuracy for the detection of cognition dysfunction.
