Abstract
What does palliative care in the community look like? From its historical origins to the modern day, death has been a community event. This chapter concentrates on the evolution of palliative care led by primary care, the MDT, implementing quality improvement using the Daffodil Standards, the Lancet Commission and Compassionate Communities. We focus on a healthy death; a public health-based approach on how end-of-life care is everyone's responsibility, and how we can practically work together with our communities to support what matters most for people affected by end-of-life care and bereavement.
