Abstract

Type 2 diabetes (T2DM) results in a heavy burden on healthcare resources, and this burden is increasing due to a number of factors. T2DM often results in multiple complications, where the epidemiology has only been described once in the literature. The objective of this study was to examine the financial costs of hospital treatment of people with T2DM with respect to the existence of multiple macrovascular complications. Data were abstracted from the Cardiff Diabetes Database for the year 1996. Costs were attributed using a statistical costing technique (healthcare resource groups (HRGs)). Costs were UK 2000 prices. For the 10,287 patients identified, 28% had at least one admission. The total estimated cost of these admissions was £7.4M ($12.6M), a mean cost per admission of £1,202 ($1,971). Mean hospital costs increased with increasing number of complications. The crude mean annual cost for patients without complications was £398 ($653), whilst for those with three complications the crude mean annual cost was £2,116 ($3,470). Mean cost for those with one complication was £999 ($1,638) and two complications was £1623 ($2,661). After standardising for age, the mean cost for those with none, one, two, and three complications was £434 ($712), £999 ($1,638), £1641 ($2,691) and 2,462 ($4,038) respectively. Of patients identified with diabetes 5,618 patients were seen as outpatients in a total of 22,100 outpatient appointments. The estimated cost of these consultations was £1.6M ($2.6M). The purpose of this study was to investigate the relationship between multiple complications and hospital costs. Hospital costs increase in a linear fashion depending upon the number of complications. They increase from an age-standardised mean annual cost of £434 ($712) without a complication to a six-fold increase in annual costs of £2,462 ($4,038) per year for the most severely ill patients with three macrovascular complications
