Abstract
To the Editor: Zongo et al suggest in their recent article that serum S100B has a high negative predictive value to rule out significant head injury in a large patient sample. Our previous work demonstrated that although there was an association between neuronal injury (acute stroke) and increases in S100B, there is clear evidence that hematopoietic cells were the main source of S100B in humans, not injured neurons. Other studies have cast doubt on the usefulness of S100B in human mild head injury as well. In our opinion, the lack of specificity of S100B for head injury or other neuronal injury (for example, stroke) means that its usefulness in this field will be limited. The time taken to perform the assay, compared with the ready availability of cranial imaging in most developed emergency medicine services, means that the central role of imaging after rational clinical assessment is unlikely to be challenged by this study.
